It is sufficient for the captain to certify that he/she has completed the form truthfully. A digital signature is not required, but if one is used, it is not a problem. However, we validate based on the Yes/No answer to “I declare that the particulars and answers to the questions are true and correct to the best of my knowledge and belief”.
